Well there are two problems ... The first more or less describes both.

She needs to stop paddling like lief and gearing up like him!

Put on da PFD!

You know. I think it makes it even worse that there were... Six? Seven? Other people who didn't notice either?!??!

There were seven of us that did that stretch, and not one of us noticed. That makes us all partially messed up. Getting bit by that "routine" bug is dangerous.

Good catch James, it's not rocket science but allot of us missed it.  The messed up part is that Ambre swam on that run and we still didn't notice.
In fact it was the next day she figured it out.  Complaceny is what gets people hurt in sports.  In climbing its not doubling back your harness buckel, rapping off the end of the rope, or a thousand other things that you screw up because you've done it before.
It's not the most flattering thing for me to post this thread, but I wanted to share the wake up call I recieved with everyone.
Just cause it's the WS doesn't mean its not dangerous!!

There was a good article in National Geographic Adventure about how we form mental models for tasks and how that relates to a number of things, including taking risk and survival. Anyway, had a good story in there about a very accomplished female rock climber in France doing a realtively easy route. In the process of preparing to climb she was carrying on a conversation with someone and was distracted. She stopped to tie her shoe and then began to climb. I don't remember is she slipped or was rappeling down, bottom line her harness wasn't tied off properly and she fell something like 75 feet. There was a convienent pine tree to break her fall, and she walked away from it. Point is, her routine that her mind went through automatically knew it was tying something, and the simple act of tying her shoe was enough subconsicously to check the box. The risk mitigation is simple, and most people do it, but the last thing you should do is take a deep breath and check your gear right before you put that first foot in the boat; then give the people to your left and right the once over. I say this as a fairly new boater, but with a lot of experience at mitigating risk on a nearly daily basis. Example -  We go through no less than four checks before we jump, your buddy checks your gear, you check it, the jump master checks it, and you check it again in the airplane before you go out the door. Guess what? Every year there's still fatailties on drop zones.

James,, I think you'll be ok,, I'll  smooth things over for ya!

I have rapped off the end of my rope.. once  its all it took, I only fell about 10 feet but it made quite an impression on me.
I have always been in the habit of looking at everyone else and performing the same check I do on my self. I have caught unfastend helmets, grab loops tucked in.
  I guess we are human?

Tobin I feel terrible I didn't notice - I was the one who was with her when she was getting ready!!  I guess that time to wait before you guys got there was enough to through the mind off track.  Shouldn't happen, but thankfully she was okay.  Even swimming down the river she seemed to stay up without any problem.....good thing it wasn't me
